JOB SUMMARY

The Social Service Specialist /Community Health Worker will be responsible for promoting public health, ensuring residents can access services, and improving the well-being of communities. The Social Service Specialist/Community Health will be responsible for performing outreach and clerical duties. As a member of the public health outreach team, the Social Service Specialist/Community Health will provide health information and services to Wayne County residents and ensure that individuals are connected to appropriate care and service agencies. Traveling to other sites such as schools, homes, festivals, businesses, and housing communities is expected of this position.

JOB QUALIFICATIONS

  • High school diploma or equivalent;
  • At least four (4) years of experience in community health, including working with diverse cultures.
